Ideally grind it down to bare shiny metal where the rust. Then prime and paint (either the whole panel or touch-up) Alternatively a rust converter could be used with some slight rust remaining if it's hard to grind off. You need a good rust converter, the best one that I knew of was banned in California at least, not sure if it's available elsewhere in the US.

1st step is to closely examine the entire car. Jack it up and check everything.
Under the plastic rocker panel covers also.

For the rust spot in the pic, remove the rear bumper cover and see how much actual rust is there.
There could be much more rust underneath. Plus check the other side also.

Once you have determined how much rust the car has, then you can make a decision on how to handle it.
